Central Community Care Access Centre (Central CCAC) provides an access point to health and personal support services to help individuals live independently at home or assist them in making the transition to a long-term care home.
This joint initiative of health and support service agencies in the Central Local Health Integration Network serves seniors 65 years and older, or over age 55 with age-related disorders, and their caregivers and families. Through a single telephone number,1-866-626-0222, assistance is provided to access services older adults need to live safely and independently in their own communities.
