New partnership to support ALC patients

North York General Hospital (NYGH) has partnered with Bayshore Home Care Solutions, a community care agency, and Queens Estate Retirement Community to provide transitional care. This model of care promotes integrated care in an environment that enhances independence with the aim of returning seniors to their home post-acute care.

“This model of care supports older adults to achieve their wellness goals, with a focus on restorative rehabilitation in a home-like setting,” says Susan Woollard, Interim Vice President, Clinical Programs. “Our aim is to help these patients regain their independence and resume normal activities. Health professionals can make this transition as seamless as possible by enhancing their abilities and focusing on their strengths.”

The 20 Central Local Health Integration Network (LHIN) funded transitional care beds are set to open at Queens Estate Retirement Community, a new retirement home at 265 Queens Drive inToronto, and are available through to April 2019.
