North York General Hospital has a strong tradition of providing innovative and compassionate care to the elderly. The community we serve has one of the highest percentages of older adults in the country.

The Seniors' Health Centre (SHC) coordinates both long-term care and the hospital's Specialized Geriatric Ambulatory Services, (outreach services, elders' clinics including chiropody services). Specialized Geriatric Ambulatory Services is part of the Regional Geriatric Program of Metropolitan Toronto (RGP), an umbrella organization that plans and coordinates specialized geriatric services. These specialized outpatient geriatric services are offered at the Seniors' Health Centre, and at the General and Branson sites.

Our long-term home is located in the Seniors' Health Centre. Residents of the 192-bed home live in an environment that supports personalized care.

 









The Seniors' Health Centre

Working in Partnership

Seniors' Health Centre staff works closely with services available in our community. We assist families to access the care they need for their loved one, whether within the hospital or in the community. The Seniors' Health Centre works in partnership with regional networks including the Central Community Care Access Centre, Doorways to Care, family physicians, health professionals, home support services, multicultural organizations and others dedicated to the health and well-being of seniors.

Launched in summer 2008, Doorways to Care is a joint initiative of health and support service agencies in the Central Local Health Integration Network serving seniors 65 years and older, or over age 55 with age-related disorders, and their caregivers and families. Through a single telephone number,1-866-626-0222, assistance is provided to access services older adults need to live safely and independently in their own communities. The service is based at the Central Community Care Access Centre office, 45 Sheppard Avenue East, Toronto.
