Features of our Diabetes Programs

North York General Hospital has two unique diabetes centres located at our Branson Ambulatory Care Centre:

Both centres enable and empower patients with diabetes to gain the self-management skills needed to achieve the best possible health outcomes in a supportive and caring environment. A referral from a primary care provider (PCP) is required for both the DEC and the CCDC. All referrals are triaged and prioritized by a diabetes educator to ensure that patients are directed to the program best suited to their individual needs.
