The Graham & Audrey Rosenberg Family Cardiac Care Clinic is an outpatient clinic that provides intervention and education to patients with congestive heart failure.
The heart function nurse practitioner and coordinator monitor your progress, provide telephone support and communicate with your cardiologist and family physician regarding your progress. Our goal is to improve the patient’s quality of life through treatment, education and support. This allows patients to recognize a problem before their conditions worsens and reduces the risk of hospitalization. The clinic can provide follow-up care to maintain the proper functioning of your pacemaker.
Our team consists of a physician, nurse practitioner, registered nurse (heart function coordinator), clinical dietitian and a pharmacist working together to provide a detailed assessment, treatment and an educational plan.
The Supportive Cardiology Program is a specialized cardiac program for people living with advanced heart failure. This program provides an added layer of support for you, your family, and your cardiology team that focuses on improving quality of life. This program is designed to reduce hospital admissions and emergency room visits by connecting you with community resources to manage symptoms at home. We offer assistance with advanced care planning and provide counseling and support to help achieve your goals.
Health care team
The supportive cardiology team consists of specialized physicians, an advanced practice nurse, and a pharmacist. This team has expert knowledge in treating symptoms common to heart failure which includes pain, shortness of breath, swelling, fatigue, anxiety and depression and will work closely with your doctors (cardiologist, family doctors, etc.). Your cardiology team will continue to treat your heart failure, which may include medications, and your supportive cardiology team will provide additional symptom management support.

A physician referral is required for the Supportive Cardiology Program. Please talk to your family physician or cardiologist.
Heart failure is a chronic illness that can negatively affect your quality of life. As the heart becomes weak, it is unable to properly pump blood and you may experience shortness of breath, pain and feeling tired.
Sometimes the heart can become too weak for medications to work causing symptoms to appear more frequently which may limit your ability to function at your best. Your cardiology team may recommend adding supportive cardiology to your care plan. The supportive cardiology team are experts in managing challenging symptoms of heart failure and strive to improve quality of life in the comfort of your own home.
The supportive cardiology team will work alongside your existing care team which may include the Cardiac Care Clinic team at North York General Hospital. Care is provided at the clinic and over the phone. The team will also connect you and your family with community resources to reduce emergency department visits and/or hospital admissions.

Information related to specific tests or treatments may include: stress tests, angiograms, coronary angioplasty, bypass surgery and permanent pacemakers.

Telemetry Unit
North York General Hospital provides care for individuals with heart disease, and many of our patients require telemetry. Telemetry is a portable remote control heart monitoring device that reads your heart rhythm and rate and relays this information to your nurse and to the central monitors. Many services are available directly on the 6th floor (West), and specialized diagnostics are provided through Diagnostic Cardiology Services.
