Please note: Some of our North York General Hospital services are now using Ocean eReferral. Faxed referrals are accepted however, do have a longer processing time than eReferral.
Using eReferral will allow us to efficiently communicate directly with you about the status of your referral.

Both centres enable and empower patients with diabetes to gain the self-management skills needed to achieve the best possible health outcomes in a supportive and caring environment. A referralfrom a primary care provider (PCP) is required for both the DEC and the CCDC. All referrals are triaged and prioritized by a diabetes educator to ensure that patients are directed to the program best suited to their individual needs.

• Patient has met established goals or goals are in progress
• Transition plan is reviewed with patient
• Referral is made
• Updated medication provided to patient
• Discharge date
• Transition to adult diabetes care initiated
