Are my symptoms COVID-19? Use our online self-assessment

North York General Hospital, Humber River Hospital,  Sunnybrook Health Sciences Centre, and Ontario Health have partnered with TELUS Health to provide residents in Toronto with an online self-assessment tool for COVID-19.

This virtual screening tool provides Torontonians with a single point of access for timely and trusted screening for COVID-19. The tool has recently been enhanced with direct integration to virtual health care providers. This will allow Toronto residents to access personalized guidance based on their individual symptoms.

The Covid19toronto.ca self-assessment tool starts with an online questionnaire designed to assess risk for COVID-19 based on symptoms. Individuals will also have the option to conduct a live virtual triage with a registered nurse online. If necessary, individuals would be directed to their local emergency department or assessment centre.

The screening tool is designed to help individuals gain access to information and support quickly and can help avoid unnecessary visits to assessment centres and supports people’s efforts to stay home as much as possible. 

The data and analytics gathered through the tool will support planning, tracking and prediction. The screening tool and associated registered nurse triage is available in English and French.
