New primary care clinic established as part of Branson transition

A message to our community:

Over the last few months, the hospital has been informing the community of the closure of the Urgent Care Centre as of June 1, 2017, and promoting the health care options currently available to the Branson community. 

Even though 99% of the patients visiting the Urgent Care Centre identified they have a family physician, we understand that many do not have same day access to primary care, or access to multidisciplinary teams. As such, a taskforce led by North York General Hospital's Department of Community and Family Medicine was established to plan further enhanced access to primary care, in a “medical home” model, in the North York communities of Westminster and Branson.

As the planning continues for a robust primary care model in the Westminster-Branson area, immediate primary care access will be facilitated in a new clinic located in the Branson Ambulatory Care Centre. As of June 1, 2017, the new Westminster-Branson Primary Care Clinic will provide an additional resource until a comprehensive primary care model is established. The clinic will allow those who are not able to gain access to their own family physician in a timely manner to gain quick access to a family physician close to home.

In addition, we are working with the Central LHIN to explore the potential for future innovative partnerships under a health hub model in that community, where primary care can be co-located with valuable community-based services like home care, mental health programs and diabetes education.
