Information for Patients about Virtual Care

COVID-19 is impacting Canada’s healthcare system. In order to protect patients and health care providers from the virus, more health care options are being offered to help limit the number of people who need to be seen in-person. Many clinics at North York General Hospital (NYGH) now offer telephone and virtual appointments to care for patients safety. NYGH has video and audio technologies for most patient visits, which avoids patients needing to come in-person. In some cases, your health care team may ask you to visit a hospital or other health care facility if necessary, for a physical examination.

Remote Patient Monitoring Virtual Care Program - New

North York General has launched a new virtual care program aimed at protecting patients and preventing hospital readmissions through remote monitoring. The new North York Toronto Health Partners (NYTHP) CareConnect program utilizes an app called Vivify, which helps monitor  patient populations: Depression and Anxiety for Mental Health, Falls Prevention for Frail and Elderly and patients with COVID positive patients.

The purpose of this new virtual care application is to decrease the number of recurring visits to the Emergency Department through enhanced symptom monitoring and rapid care for both those with mental health concerns and frail and elderly patients who are more prone to falls.
