Remote Patient Monitoring Virtual Care Program

North York General Hospital has launched a new virtual care program aimed at protecting patients and preventing hospital readmissions through remote monitoring. The new North York Toronto Health Partners (NYTHP) CareConnect program utilizes a remote patient monitoring app, which helps monitor COVID-19 positive patients who are aged 12 and older. Other patient populations can also enroll into the Remote Patient Monitoring Program, including:

  • North York at Home (home care program)
  • Surgical Transitions

The purpose of this virtual care application is to decrease the number of recurring visits to the Emergency Department through enhanced symptom monitoring and rapid care.

** Physicians and NYGH units may refer those who are not COVID positive into the program via Cerner.

This initiative received provincial funding to enhance the pandemic response through appropriate virtual care options, which are key in ensuring continuity of health services while preventing the spread of COVID-19.
