Connected Care
In 2024/25, North York General focused on bolstering our partnerships with other health and community organizations to improve access to care through seamless referrals, timely information-sharing, and by bringing care into people’s neighbourhoods and homes.
We worked with the North York Toronto Health Partners (NYTHP) and our area’s Primary Care Network to introduce a hub model for managing chronic conditions, such as diabetes. Digital health has also assumed an increasing role in connecting people with personalized care throughout their journey including in the community after they leave the hospital

Community Health Information Fairs Connect People to Services and Information

Teresina Stanichevsky is a long-time volunteer and advocate for newcomers to Canada. As the Lead Community Ambassador for our Ontario Health Team, NYTHP, she connects people to health care, community and social services and other information at Community Health Information Fairs (CHIFs). NYG is a proud core partner of the NYTHP and contributes services, staff and space to support the CHIFs.
Transforming Care through Digital Health
Since its inception in 2021, NYG’s five-year Digital Health Strategy has enabled NYG to drive measurable improvements in patient care. Innovations including remote patient monitoring, online access to medical records and an app to help patients and caregivers seamlessly navigate hospital and healthcare services, have made health care safer, more seamless and responsive for patients and health professionals.

We were the first hospital in Canada to move our electronic health record (EHR) to the cloud, which has made it more reliable and more secure against cyberattacks. For a patient to have all their health information in one system that can be accessed by all providers, regardless of whether the patient is visiting a clinic or is hospitalized, is important from a patient safety standpoint.
