Birth registration and child benefits

All births are registered online. You will receive a card with the web address. If you do not have access to a computer, you can call 1-800-267-8097 to find out where the closest Service Ontario is located. Births must be registered within 30 days. You can apply for a birth certificate and a social insurance number at the same time you register your baby’s birth. You will receive a form to apply for the Ontario child benefit and the Canada child benefit. Please send the completed form in the envelope provided.

Use the 5 in 1 to:

  • Register your child’s birth
  • Apply for birth certificate
  • Apply for a social insurance number (SIN)
  • Apply for Canada Child Benefits (including the Ontario-specific child benefits)
  • Request an Education Savings Referral

OHIP application

All moms with a valid OHIP number will receive the Ontario Health Coverage Infant Registration form for their baby from a hospital staff or midwife. On the form, a custodial parent or legal guardian who resides with the baby must:

  • Give your baby’s name, birthdate and mailing address
  • Confirm your baby’s primary place of residence is Ontario
  • Confirm your baby will be in Ontario for at least 153 days in any twelve-month period in order to retain OHIP coverage

It is very important to make sure that the name you put on the OHIP application is the exact same name you use when you register the baby’s birth. If you have not chosen the baby’s first and middle names, print the last name on the registration form and leave the first and middle names blank. You will receive a form requesting your baby’s full name before a health card can be sent.

It is vital to complete this form before you leave the hospital. The form has a number on it that is recorded, so it is critical to make sure that the form is not misplaced or lost. The hospital or midwife will submit the top part of the form to ServiceOntario on your behalf.

Keep the bottom section of the form until you get your baby’s Ontario health card. It shows the health number assigned to your baby. Your baby’s new health card will be mailed to you within eight weeks of the date the Ontario Health Coverage Infant Registration form was submitted.
