To sign up for MyChart™, we first need to verify your identity. To sign up, you must fill out our online registration form. If you already have a myChart™ account, login below:
Please call our Health Records Release of Information department and leave a voicemail and we will call you back within 48 hours. They will guide you on how to get access on behalf of a patient: 416-756-6209. You can also email patient.records@nygh.on.ca.

Frequently Asked Questions
My Chart is an online website where patients can create and manage their own personal health information based on clinical and personal information.
Patients have access to their own MyChart™ account (which includes information from other hospitals participating in MyChart™), but they can electronically grant access to family caregivers, hospital clinicians, primary care physicians, pharmacists, or other members of their care team.
MyChart™ is accessible from anywhere, at any time, through the Internet. Once you have a PIN, and registration is complete, users can go to MyChart website (external website) and log in to review their MyChart records.
